Ord Prima is a planet in the Outer Rim Territories. It has a harsh but habitable atmosphere, and while it's large oceans contain elements that make them poisonous to humans, it's plentiful lakes can be purified for drinking and irrigation. A small farming colony occupied the primary continent from 12BBY to 3BBY.
In 3BBY the governor of the colony provided temporary refuge to the crew of a small ship carrying a known commander of the Rebel Alliance. The 51st Storm Commando tracked them there, observing from a distance. Lt. Colonel Randus Parr decided to execute the rebels and three-hundred colonists in retaliation for their cooperation.
In the mission, Captain Fein "Cage" Dorn was assigned by to detonate thermal charges under the structure that housed a number of people, including the rebel contingent. The charges were placed, but at the last moment he made the decision to defy the order and did not detonate the charges.
Major Rafe Aquilla was given the order to lead the main body of commandos into the compound. When the final order was given to advance, he refused, ordering the men with him to stand down while a much smaller unit advanced on the colony from the west. By the time Aquilla was relieved of command and his replacement given the order to advance, the colony had been alerted.
As a result of Lieutenant Dorn's failure to detonate the explosives and Major Aquilla's failure to advance when ordered, all of the rebels were able to escape. Unfortunately, other than some outlying farms, the entire population of the colony was executed as planned.
